About Your Member ID Card
When you enroll with Group Health, you and each member of your family covered under your plan receive a member identification card.
What's On Your Member ID Card
At the top, you'll find your name, member ID number, and health plan name.
- Use your member ID number to make or cancel appointments, check in for appointments, order or pick up prescriptions, get laboratory or X-ray services, receive emergency care, and log in to MyGroupHealth for Members.
- The health plan name (also called your provider "network") indicates which doctors and other health care providers are covered under your health plan. You'll need this information when using our online provider directory.
At the lower left are numbers that help us process your pharmacy claims.
The Alliant and Options health plans include out-of-network care through Beech Street, First Choice Health, or both networks. The plans also may allow use of out-of-network pharmacies via the MedImpact network. Logos on the lower right of your Alliant or Options plan cards indicate which out-of-network care is allowed.

Healthy Options Members
If you are in the Healthy Options plan network, you should also have a Department of Social and Health Services (DSHS) medical identification card. See a sample card. In early 2010, DSHS will replace its medical ID card with the ProviderOne card. For more information, see the DSHS ProviderOne Web site.

On the back of your ID card, you'll find helpful contact numbers for Customer Service and our Consulting Nurse Service, as well as a place to write your doctor's phone number.
2009 Group Health and Clear Care Cards
For the 2009 cards, if your card does not include a health plan name, you are enrolled in either our Group Health or Clear Care (Medicare Advantage) plan. The Clear Care card includes a CMS number on the lower left.
