Living Well With Chronic Conditions In-Person Workshops

These six-week in-person, group workshops help people learn how to manage their chronic conditions and improve their quality of life. Participants learn to improve their health one step at a time by setting goals and then breaking the goals into small steps, week by week. Workshop participants support each other and work together to solve their problems.

Two specially trained volunteers lead the workshops, at least one of whom has a chronic condition. Participants are encouraged to attend these workshops with a partner or caregiver.

Registration

Register for an in-person workshop. There is no charge for the workshop, but we ask each person or family to pay what they can toward the cost of the reference book. (Only one copy per family is needed.) Financial assistance for the book is available on request.

Note: Occasionally a workshop needs to be rescheduled. Contact the medical center where the workshop will take place to confirm the date, time, or location.

Volunteer Opportunity

These workshops are led by pairs of trained volunteer leaders who follow a structured curriculum that includes learning sessions, brainstorming, skill building, and group sharing. Living Well leaders have the satisfying role of facilitating the workshops in a way that engages participants and has a life-long impact.

As a leader, you'll learn group facilitation skills that can be transferred to other areas of your life. Most importantly, you'll make a positive contribution that results in participants feeling healthier and more confident as they manage their conditions.

To qualify you need to:

  • Have a chronic health condition or care for someone who does
  • Successfully use skills learned from taking a Living Well with Chronic Conditions workshop
  • Be able to attend leader training that takes place over four full days
  • Commit to co-leading at least one six-week workshop within the first year of training
