Health Profile and Wellness Services

Get a jump on better health with Group Health's wellness services: The online Health Profile and a Lifestyle Coaching service, as close as your phone, to assist you in making lifestyle changes. These convenient services are available to all adult Group Health members at no additional cost.

Your Health Profile

The Health Profile is an online questionnaire and report to help you take better control of your health. You can use this health risk assessment as a guide in discussing your overall care with your doctor.

In the questionnaire, you'll be asked about your medical history and your lifestyle routines. If you get your primary care at a Group Health medical center, certain information from your medical record already will be included, such as your most recent blood pressure reading and cholesterol levels.

After you submit the questionnaire, you'll receive an online report with your health status, your risk for certain diseases, and suggestions for improving your health. When you update your profile — every year, or more often if you'd like — you'll see a comparison to what's changed since your last profile.

To access the Health Profile, you must register  with MyGroupHealth for Members. The next step is to register for MyGroupHealth for Members so you have access to your personal online services, including the Health Profile. After you complete the registration process, you'll find a link to the Health Profile on your MyGroupHealth for Members home page.

Lifestyle Coaching

Sometimes you need more information or a little support in making lifestyle changes to improve your health. A coach can help you make positive changes in your eating habits, physical activity, and tobacco use.

Depending on your answers in the Health Profile questionnaire, and whether or not you provide your phone number, a coach may contact you.

The coaches are health care professionals, such as nurses and dietitians, who are trained in helping people make behavior changes. Through ongoing phone conversations, a coach can help you stay on track and reach your own health goals.
