Home and Long-Term Care
Group Health provides help and support for those who require long-term care or home health care. These services include skilled nursing and physical therapy at home, hospice services, coordination of nursing home care, and at-home social work care. Coverage for these types of services is dependent upon the enrollee's medical coverage agreement.
Home Health Services
If you or someone you care for is temporarily unable to leave home because of health problems, Group Health's Home Health Services can bring skilled nursing care, physical therapy, and occupational therapy to your home. Whether you're recovering from an illness or accident, or learning to manage a chronic health problem, Home Health Services will help you work toward returning to independence.
The Home Health Services team includes registered nurses, physical therapists, occupational therapists, speech pathologists, medical social workers, and home health aides. These health care professionals help patients learn about walking aids, special diets, medications, and appropriate exercises to regain or retain strength. Hospice Program
Group Health's Hospice Program offers physical, emotional, and spiritual care for patients who are in advanced stages of incurable illness.
Nursing Homes
The decision to admit to a nursing home is made by your physician. Admission can occur from the hospital, emergency department, or clinic. Covered skilled nursing care is usually provided in Group Health-contracted facilities.
