A patient-centered medical home offers personalized care, when you need it, for the majority of your health care needs. At your Group Health medical home, the members of your care team have access to the best scientific research available to guide them in making recommendations about your care.
Group Health's medical centers have been recognized by the National Committee for Quality Assurance (NCQA) as Level 3 Patient-Centered Medical Homes. This means our medical centers offer exceptional practices to make sure our patients receive the best care available, based on the latest scientific research. It also means our patients' treatments and care are coordinated among all providers involved in their care.
What Does Medical Home Mean for You?
A team approach. You and the members of your health care team will work together to make decisions about your health. In addition to your doctor, your team might include a physician assistant, advanced nurse practitioner, and nurse. You might also have a care manager and other health care professionals. Together, all members of your team will make sure you get the tools and support you need, including Behavioral Health Services, patient and family education, self-management support or care from other qualified professionals.
A strong relationship with your personal health care provider. Your team knows you and your medical history, and works with you to take charge of your health care. Giving your care team accurate information about your medical history and about care you've had outside of Group Health will help your team be even more effective. Whether you have a new problem, are focused on preventive care, or are managing a chronic illness such as diabetes or asthma, your team will support you and make sure you have access to the services you need.
Access centered on your needs — through office visits, phone visits, and secure e-mail. Between office visits, your team may check in with you to see how you're doing or answer any questions you might have. Many times, a member of your health care team will call you, rather than waiting for you to call about a problem. Your medical record and health history are available to all of your Group Health providers. This helps them in making decisions about your care. If you have any medical concerns after clinic hours, Group Health's Consulting Nurse Service can access your medical record and give you clinical advice and care instructions.
A focus on prevention. Your doctor and care team will talk to you about, and explain, any screening tests you need. Screening tests help find risk factors or early signs of illness. You'll discuss treatment options that can help slow or prevent illness. And you'll talk about any lifestyle changes you want to make and how to make them.
